Sickle Cell Disease Enhanced Care Management

Sickle Cell Disease Enhanced Care Management, LLC (SCDECM) connects individuals living with sickle cell disease (SCD) to comprehensive Enhanced Care Management (ECM) services as defined by the Department of Health Care Services.

Through partnerships between community health workers and clinical teams that specialize in the care of persons with SCD, SCDECM ensures patients receive coordinated, wrap-around support that addresses both medical and social determinants of health.

As a subsidiary of the Center for Comprehensive Care and Diagnosis of Inherited Blood Disorders (CIBD), SCDECM extends CIBD’s mission by bridging the gap between clinical treatment and community-based care coordination, facilitating continuity of care.
