About Us

Our History

Sickle Cell Disease Enhanced Care Management (SCDECM) originated from the Networking California for Sickle Cell Care (NCSCC) initiative, a collaborative effort among California’s sickle cell treatment centers to improve care coordination and outcomes for adults living with sickle cell disease.

As the NCSCC program demonstrated the impact of dedicated case management services for this high-need and underserved population, it became clear that a sustainable reimbursement mechanism was essential to maintain these services beyond grant funding. SCDECM was developed as a response to this challenge, leveraging California’s Enhanced Care Management benefit under (California Advancing and Innovating Medi-Cal) CalAIM to ensure ongoing access to comprehensive care coordination for sickle cell patients.

Partnership with CIBD and SCDF

Sickle Cell Disease Enhanced Care Management is proud to partner with the Sickle Cell Disease Foundation (SCDF) to deliver comprehensive Enhanced Care Management services to individuals living with sickle cell disease throughout California.

Through this collaborative partnership, SCDECM and our clinical partners work together to provide whole person care that addresses not only medical needs but also the critical social determinants of health (SDOH) that impact our members’ wellbeing. SCDF’s expertise in supporting the sickle cell community, combined with our clinical partners’ medical expertise, creates a care model that addresses:

  • outreach and engagement,
  • comprehensive assessments and care management,
  • enhanced coordination of care,
  • health promotion,
  • transitional care services,
  • member and family supports, and
  • coordination of and referral to community and social support services.
